Siam Kitchen, a takeaway at 353 Woodstock Road, Belfast was given the score after assessment on August 2, the Food Standards Agency's website shows.
It means that of Belfast's 395 takeaways with ratings, 207 (52%) have ratings of five and none have zero ratings.
