Raffo's Pizza and Ice Cream, a takeaway at 794 Springfield Road, Belfast was given the score after assessment on April 26, the Food Standards Agency's website shows.
It means that of Belfast's 393 takeaways with ratings, 194 (49%) have ratings of five and just one has a zero rating.
